Steer Health A4M Rewards Program

1. Program Overview

The Steer Health A4M Rewards Program is offered exclusively to attendees who visit the Steer Health booth at A4M Longevity Fest 2025 and book a qualifying demo. Participants who complete their demo by the specified deadline will receive certain rewards and be entered into a giveaway, as outlined below.

2. Eligibility

2.1. The Program is exclusive to A4M Longevity Fest 2025 booth attendees who submit their details in person at the Steer Health booth.

2.2. Participants must be licensed clinicians, medical practice owners, or authorized clinic staff working in functional medicine, integrative medicine, longevity or related fields.

2.3. Participants must book their demo on or before December 14, 2025.

2.4. Participants must attend and complete their demo on or before December 31, 2025.

2.5. If a Steer Health representative is unavailable for the selected slot, an alternate date beyond December 31 may be offered upon request at Steer Health’s discretion.

2.6. Only participants with a valid business/clinic email address may enter. Gmail/Yahoo/Hotmail or other non-business personal emails may be rejected for eligibility.

4. Demo Completion Requirements

4.1. A demo is considered complete only when the participant has engaged in a minimum of 15 minutes of active participation with a Steer Health representative.

4.2. Steer Health reserves the right to deny rewards if the participant:

5. Fair Participation Rules

5.1. One demo per clinic email domain. Multiple entries from the same clinic or domain may be disqualified unless clearly representing separate qualified staff members.

5.2. No group walk-ins may register as a single entry. Each participant must individually complete the form and attend the demo.

5.3. Participants may not create multiple identities, emails, or false clinics to obtain additional rewards.

5.4. Steer Health reserves the right to request proof of affiliation with the clinic listed.

5.5. Steer Health may disqualify any participant suspected of violating these T&Cs without notice.

6. Reward Fulfillment

6.1. SteerNotes AI Scribe access will be activated within 5 business days after the demo.

6.2. Amazon Gift Cards will be emailed to the verified participant within 10 business days after demo completion.

6.3. The iPad 11 giveaway winner will be selected at random from all qualified participants and notified by email.

If no response is received via email within 7 days, Steer Health reserves the right to select an alternate winner.

7. Limitations & Disclaimers

7.1. Rewards are non-transferable, non-exchangeable, and may not be redeemed for cash.

8. Privacy

8.1. Participant information will be used solely for demo scheduling, reward fulfillment, and product communication purposes as permitted by Steer Health’s privacy policy.

8.2. No participant data will be shared with external parties unrelated to program fulfillment.

9. Acceptance of Terms

By registering for the Program, participants acknowledge they have read, understood, and agreed to these Terms & Conditions.
